BOYS REGISTRATION

You have 3 options to register your team :

a) MAIL:    Print the registration form ; complete it ; mail it to our office address on the form.

                  Registration fee of $750.00 must be included. No post dated cheques please.

b) FAX :    Print the registration form ; complete it ; fax it to our office (613 ) 732 - 1370

                 Mail a copy of this registration along with your fee of $750.00 to the address on the form.

                 We will only process this registration when the fees are received.

c) ON-LINE :    You need Microsoft Word 2003 or later to use this feature.

                    INSTRUCTIONS:

                                If you have Office 2007

                                  - complete the on-line form, then click the Office button ( top left )

                                    and send as an E-mail attachment to:    silverstickpembroke@hotmail.com

                                  - Registration fee of $750.00 must be sent to the address on the form.

                                  - Ensure the check identifies your team name and division



Other versions of Word may require you to save the document first and then email it as an attachment.
